Before you come to Foundation Surgical Hospital, your surgeon will review with you the details of your surgery and you will be given detailed instructions. The following forms may be completed prior to your procedure. Additional forms may need to be completed at the hospital.

The Patient Bill of Rights and Responsibilities explains what actions we need from you as a participant in your care, and what behavior you can expect from us. A successful recovery is based on mutual understanding of what each participants' roles are. You will be given this form at registration and can also access it by clicking on the link above.

Scheduling Your Surgery

You will be given a tour of our facility and a pre-operative educational meeting where you will learn what to expect before, during, and after your surgery. Our goal is to provide information to prepare you for surgery and ease any anxiety you or your family members may experience during the healing process. Topics discussed include where to come the day of surgery, pain control, anesthesia, respiratory therapy, equipment used during and after the hospital stay, and discharge planning.

We will ask you to pre-register in the business office upon arrival to the hospital to make sure everything is complete for the morning of surgery. We encourage family/caregivers to take part in the pre-admit meeting so they can be of utmost support to you during your recuperation.

Prior to the class you will see one for our pre admit nurses for an assessment, lab work, and any other testing ordered by the surgeon or anesthesiologist before the surgery.

Oasis and International Program

At FSH, we offer a program for both the international and local patients who desire individualized support and special concierge services during their stay. These services include appointment scheduling, language assistance, flight arrangements, hotel and transportation assistance as well as any other need during your stay. The Oasis Unit has eight patient suites with special accommodations for the comfort and privacy of our patients and their families.
